NNPHI Launches Phase 2 of State Forums to Advance Health Systems Transformation
The National Network of Public Health Institutes (NNPHI) recently awarded IMPH funding to support an initiative to bridge public health and health care. Supported by the Robert Wood Johnson Foundation, this award will allow IMPH to convene stakeholders in a state forum to explore tangible strategies for fostering collaboration between these two sectors. The focus of the forum and related work will be on defining the health care workforce of the future and its role in enhancing overall population health.
“In the age of health care reform, it is imperative that the health care workforce shift from the current illness-focused model to one centered on prevention and health promotion,” said Maya Pack, IMPH Associate Director for Research and Strategic Initiatives. “Through this grant, IMPH will begin a process that will identify collaboration and policy opportunities to enhance the future health care workforce in South Carolina. We appreciate the opportunity to collaborate with NNPHI and fellow public health institutes in this process.”
Other public health institutes awarded the grant include:
- Hawaii Public Health Institute will establish a Rural Community Health Ambassador Program to engage rural communities in current state-wide health system transformation efforts.
- Illinois Public Health Institute will engage Medicaid and private payer managed care plans and hospital systems to leverage community-based chronic disease prevention and self-management programs.
- Mississippi Public Health Institute will replicate the work of the Phase 1 grantee Health Policy Institute of Ohio to create a Mississippi Health Value Dashboard with a focus on behavioral health.
- Public Health Management Corporation (Pennsylvania) will replicate the work in Virginia by the Institute of Public Health Innovation to integrate community health workers into the healthcare system to support population health improvement.
- Tennessee Institute of Public Health, in collaboration with Healthy Appalachia Institute, will support the development of an Accountable Care Community with a health system that operates in Northeast Tennessee and Southwest Virginia.
