SC Coordinated Chronic Disease Prevention and Health Promotion Plan

The South Carolina Department of Health and Environmental Control (DHEC) Bureau of Community Health and Chronic Disease Prevention (BCHCDP) utilized the Coordinated Chronic Disease Prevention and Health Promotion Program Cooperative Agreement from the Centers for Disease Control and Prevention (CDC) to facilitate development of a South Carolina Coordinated Chronic Disease Prevention and Health Promotion Plan (CCDPHP).  In close collaboration with DHEC, IMPH provided technical assistance and analysis to assist in the development of the plan, which will inform and guide BCHCDP’s chronic disease and health promotion integration process.

This work concluded in September 2014.
