The 15th Annual Chronic Disease Prevention Symposium Health Systems Transformation: Working Together to Improve Population Health in South Carolina will be held March 10-11, 2017 at the Sheraton Myrtle Beach Convention Center Hotel in Myrtle Beach, SC. The symposium will feature presentations by state and national experts on topics including: eliminating disparities in access to quality cancer care, diabetes and hypertension control, opioid use and abuse, building care coordination models to control chronic disease and learning practice transformation tips. Continuing Education Credits will be provided.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the South Carolina Medical Association and the S.C. Department of Health and Environmental Control.

The South Carolina Medical Association is accredited by the ACCME to provide continuing medical education for physicians.
